Informed consent in studies with human subjects

The institutional review board approved this study with a waiver of consent in view of the study’s retrospective nature, and all the procedures being performed were part of the standard of care (receipt number 1582). All procedures followed were in accordance with the ethical standards of the responsible committee on human experimentation and with the Helsinki Declaration of 1975 as revised in 2008.
